What You Will Do:

Review Section 117 Aftercare eligibility in line with the Mental Health Act
Complete Statement of Need (SoN) reviews to ensure appropriate care provision
Undertake Care Act assessments and develop support plans
Collaborate with NHS colleagues and multi-agency partners to deliver integrated care
Participate in multidisciplinary meetings and contribute to care planning

Requirements:

Strong knowledge of the Mental Health Act and Care Act
Experience working within adult social care and multi-agency environments
