What you will do:

• Undertake strengths-based assessments, reviews, and support planning in accordance with the Care Act 2014.
• Assist young individuals with Learning Disabilities and Autism through their transition from children's to adult services.
• Develop, implement, and review person-centred care and support plans that encourage independence and choice.
• Complete Mental Capacity Assessments and contribute to decision-making under the Mental Capacity Act 2005, including Court of Protection processes when necessary.
• Identify, assess, and manage risks while ensuring safeguarding concerns are addressed following local and national procedures.

Person Specification:

Qualifications:

• Hold a degree or postgraduate qualification in Social Work.
• Have completed the Assessed and Supported Year in Employment if applicable.
• Demonstrate evidence of ongoing Continuing Professional Development.

Registration and Compliance:

• Maintain current registration with Social Work England.

Experience:

• Previous experience within Adult Social Care is crucial.
• Have supported individuals with Learning Disabilities and/or Autism.
• Familiar with care management, casework, and statutory interventions.
