What you will do

Play a crucial role in the assessment of mental capacity, ensuring that health, wellbeing, choice, and control are prioritized and respected.
Facilitate communication and build strong person-centred relationships with service users and their families or carers, guiding them through their care processes in a compassionate manner.
Collaborate with a team from varied disciplines, including partnerships with community agencies, to enable and empower individuals to live independently and exercise autonomy.
Participate actively in both individual and team development activities to stay on top of best practices and innovative care approaches.
Implement and identify needs for assistive equipment during assessments, referring more complex cases to specialists when necessary.
