We are seeking an experienced Adult Social Worker to join the Community Mental Health Team in the vibrant city of Liverpool. What you will do

Manage a varied caseload of adults receiving support from secondary mental health services, delivering person-centred interventions and ongoing case management.
Complete strengths-based assessments, develop care and support plans, and regularly review outcomes to promote independence and recovery.
Work closely with the Transition Team, supporting young people moving into adult mental health services. Experience within Children's Social Care is desirable but not essential.
Collaborate with service users, families, healthcare professionals, and partner agencies to provide coordinated, holistic care.
Maintain accurate, timely, and legally compliant case records and documentation in line with statutory requirements and professional standards.

Requirements

Qualified Social Worker with current Social Work England registration.
Experience working within Adult Social Care, ideally in a Community Mental Health Team or similar setting.
Strong understanding of the Care Act 2014, Mental Health Act, Mental Capacity Act, and related legislation.
Excellent assessment, care planning, communication, and multidisciplinary working skills.
Ability to manage a busy caseload while delivering high-quality, person-centred practice.
