What you will do:
- Deliver comprehensive social work services to individuals on forensic mental health wards, ensuring that all regulatory frameworks and standards are met.
- Collaborate effectively with the MDT, participating in case reviews and joint planning to provide holistic and person-centred care.
- Assess and manage a caseload of patients, identifying their social care needs and implementing plans to meet those needs.
- Facilitate discharge planning, ensuring a smooth transition from inpatient care to community settings.
- Provide support and guidance to family members, involving them in care plans where appropriate.
- Adhere to and uphold policies and procedures of North London NHS and Enfield council to ensure consistent and high-quality social care.
