What You Will Do

Undertake Care Act 2014 assessments for adults referred to Intermediate Care, focusing on recovery and independence.
Develop and implement short-term care and support plans in collaboration with health professionals and service users.
Facilitate timely hospital discharges by coordinating community-based support.
Review care plans regularly and adapt them to reflect changing needs.
Carry out safeguarding enquiries under Section 42 where appropriate.
Ensure all work complies with the Care Act 2014, Mental Capacity Act 2005, and other key legislation.
Promote a preventative and strengths-based approach, reducing reliance on long-term services.
Maintain accurate records and contribute to high-quality service delivery.
