Key Responsibilities:

• Conduct thorough occupational therapy assessments for older adults with mental health needs.

• Develop and implement personalised rehabilitation programmes tailored to individual requirements.

• Participate in discharge planning, liaising with patients, families, carers, and community services.

• Collaborate within a multidisciplinary team to ensure integrated, patient-centred care.

• Maintain accurate and timely clinical records in accordance with NHS policies.

• Champion patient-centred care and support initiatives that contribute to service improvement.
