Key Responsibilities:

Conduct thorough occupational therapy assessments for older adults with mental health needs.

Develop and implement personalised rehabilitation programmes tailored to individual requirements.

Participate in discharge planning, liaising with patients, families, carers, and community services.

Collaborate within a multidisciplinary team to ensure integrated, patient-centred care.

Maintain accurate and timely clinical records in accordance with NHS policies.
