Located at Chateau Reve, you will work as part of the Adult Disability Service in a full-time, permanent position, earning £49,970 per annum. What you will do:
- Lead and coordinate a team to develop and implement high-quality care and support programmes.
- Assess, develop, implement, and evaluate individual care plans, ensuring they meet evidence-based standards.
- Work autonomously while recognising when to escalate concerns to senior staff.
- Guide, advise, and support staff in delivering excellent programmes of care.
- Advocate for patients and service users, promoting health education and safety.
- Build therapeutic relationships with service users and their networks, using effective communication skills tailored to diverse and sometimes challenging situations.
- Promote teamwork and uphold our core values to achieve the best outcomes for service users.
- Ensure records are accurately maintained and safeguard the wellbeing of service users.
