We are delighted to offer an exciting opportunity for a Qualified Social Worker to join the Adults Community Mental Health Team (CMHT) in Salford. This is a full-time, ongoing position offering an hourly rate of £30.00. You will play a vital role in promoting recovery, wellbeing, and independence through person-centred and strengths-based social work practice.

Key Responsibilities

• Undertake comprehensive, strengths-based assessments in line with statutory duties and local authority frameworks.

• Develop and implement person-centred care and support plans that promote independence, wellbeing, and recovery.

• Manage a caseload of adults with severe and enduring mental health needs, ensuring timely reviews and effective interventions.

• Work collaboratively with service users, carers, healthcare professionals, and partner agencies to deliver coordinated care.

• Exercise sound professional judgement to manage risk, safeguarding concerns, and complex decision-making.

• Contribute to multidisciplinary team meetings, case conferences, and discharge planning processes.

• Maintain accurate and timely case records in line with Salford City Council policies and procedures.

• Ensure compliance with all relevant legislation and statutory requirements, including the Care Act 2014, Mental Health Act 1983, and Safeguarding Adults procedures.

• Participate in supervision, reflective practice, and continuing professional development to maintain and enhance professional standards.

• Support the implementation of service improvements and council-wide initiatives to enhance mental health and wellbeing outcomes across Salford.
