What you will do:

• Conduct thorough assessments, rehabilitation, and discharge planning for patients with neuro conditions, ensuring tailored and holistic care.
• Manage disability in patients with deteriorating conditions, developing and implementing strategies for effective symptom management.
• Supervise and handover cases to OT assistants, fostering a collaborative care environment.
• Own and drive a car to facilitate efficient community visits and patient support.

To thrive in this role, you will need to be registered with the HCPC and have a valid driving license, as travel is a significant component of the job. Your ability to navigate community settings and apply clinical expertise will be your greatest tools on this adventure.
