What you will do:

Undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults with complex health needs.
Act as a Case Manager for clients receiving continuing healthcare funding, assessing and facilitating their care needs.
Communicate effectively with patients, families, and healthcare professionals, ensuring clear and accurate information transfer.
Develop, review, and manage care plans to meet patients’ needs, advocating for them and ensuring they receive optimal care.
Support and guide care providers, offering expert advice as necessary.
Participate in Continuing Healthcare panels, appeals processes, and service development initiatives.
Maintain accurate records and reports aligned with the NHS Continuing Healthcare and Funded Nursing Care frameworks.
