What you will do:

Conduct comprehensive bedside assessments at hospitals to determine patients' needs.
Travel to complete home assessments, empowering individuals to achieve greater independence.
Engage in care act assessments, promoting a tailored approach to support each person’s trajectory towards reablement.
Collaborate with our Homefirst Neighbourhood team to find creative solutions in the discharge to assess process.
Attend occasional meetings in the office, while enjoying the flexibility of completing paperwork from home.

As a qualified Occupational Therapist, registered with HCPC, you are required to have two years of post-qualification experience. A car and valid driving license are essential for conducting home visits and assessments.
