What you will do:
- Assess and manage the nutritional needs of individuals with learning disabilities and mental health conditions.
- Develop, implement, and monitor nutritional care plans tailored to each patient's needs.
- Provide expert nutritional guidance to patients, their families, and other healthcare staff within the team.
- Liaise with multidisciplinary teams to ensure an integrated approach to care.
- Keep precise records and documentation to track patient progress and adjust care plans as necessary.
Significant home enteral feeding experience is essential, as you will be working with patients requiring this type of support. Experience in learning disability and mental health settings is desirable to better cater to the specific needs of the individuals in your care. A valid driving licence and access to a car is also required to facilitate visits and outreach.
