What you will do:

• Conduct thorough assessments of newly referred adults to the Early Intervention Service who are experiencing First Episode Psychosis (FEP).

• Provide evidence-based psychosocial interventions focusing on engagement, recovery, and social inclusion.

• Address physical health care needs of patients to cater to their holistic well-being.

• Utilise your knowledge of the EPJS/SLAM system for effective patient management.

• Collaborate with a multidisciplinary team to develop and implement comprehensive care plans.

• Maintain detailed patient records and progress reports, ensuring compliance with HCPC standards.

Requirements:

• HCPC registration is mandatory for this role.

• Experience working in mental health teams within community settings.

• A specific interest in working with individuals with developing psychosis.

• Being a car driver is advantageous for this position, enhancing your ability to visit patients within the community.
