We are seeking a Band 6 Care Coordinator to join the Adults Community Mental Health Team in Manchester. This ongoing locum position offers a competitive hourly rate of £26.00, with flexibility to work full-time or part-time. Core hours are 9am–5pm, Monday to Friday, with options for earlier or later start and finish times—or even condensed four-day work weeks. Key Responsibilities

  • Manage a caseload of adults with severe and enduring mental health conditions.

  • Act as Care Coordinator under the Care Programme Approach (CPA), creating and updating recovery-focused care plans.

  • Complete thorough risk assessments, developing and implementing effective crisis and risk management plans.

  • Conduct holistic assessments of mental, physical, and social care needs, delivering evidence-based interventions.

  • Liaise with GPs, social workers, and other agencies to ensure integrated, multi-agency care.

  • Attend CPA reviews, safeguarding meetings, and contribute effectively in multidisciplinary discussions.

  • Respond to safeguarding concerns in accordance with Trust procedures and statutory requirements.

  • Maintain accurate clinical records in line with policy and professional standards.

  • Offer informal supervision and guidance to junior colleagues or students.
