Overall purpose of job:
To plan, provide, and evaluate a range of person-centred social work interventions to individuals and their support network, ensuring their needs are met and aspirations achieved. Flexibility in undertaking duties is required, adjusting to the evolving needs of the post.

What you will do:
- Develop and implement personalised care plans.
- Conduct complex assessments and reviews to ensure best outcomes for individuals.
- Liaise with various stakeholders to ensure coordinated care and support.
- Work on complex cases involving Deprivation of Liberty Safeguards (DOLS) and Court of Protection (CoP) matters.
- Advocate for individuals’ rights and needs, ensuring their voices are heard.

Requirements of the Social Worker:
- Degree or equivalent in social work.
- Current Social Work England registration.
- Proven frontline experience in UK social services as a qualified Social Worker.
- Experience with Learning Disabilities (LD) and Mental Health (MH) cases.
- Ability to handle complex work, including reviews and DOLS/CoP matters.
