What you will do:
- Support individuals with learning disabilities and autism in their daily living activities, ensuring their care plans are followed effectively.
- Develop and contribute to care plans in collaboration with a Registered Health or Social Care Professional, promoting health education and wellbeing.
- Recognise changes in service users' conditions and take appropriate action by referring them to the supervising professional.
- Maintain professional boundaries while fostering a caring and supportive environment for all service users.
- Work closely with younger adults to assist in managing their tenancies and help them lead independent lives with dignity and confidence.

This role requires someone who is eager to learn and capable of working both independently and as part of a team. Your work will reflect our core CARE values, which define our approach to integrated health and social care.

Minimum Qualifications:
- Possess a Band 3 competency pathway or Level 3 VQ Diploma in Health and Social Care, or
- Hold a Level 3 BTEC Advanced Certificate in Positive Behavioural Support.
- Achieve a full qualification by the second gateway at Diploma Level (Level 5 HE) or equivalent, such as Level 4 BTEC Professional Certificate in Positive Behavioural Support.
