• Conduct comprehensive reassessments of individuals within the learning disability and autism spectrum to ensure their needs are met.
• Collaborate with other professionals to devise effective support plans tailored to each person's unique requirements.
• Facilitate access to services and resources for individuals and their families, empowering them within the community.
• Maintain accurate and up-to-date case records, ensuring all sensitive information is handled confidentially.
• Provide ongoing support and advice to individuals and caregivers, promoting independence and enhancing quality of life.
