We’re seeking an experienced Adult Social Worker to join the Older Persons and Physical Disability Service in Southwark. This full-time locum position offers a competitive £33 hourly rate and the opportunity to manage a varied caseload within a progressive and supportive team.

What you will do:

• Manage a diverse and complex caseload of older persons and adults with physical disabilities.

• Carry out Care Act, Carers', Mental Capacity, and CHC assessments.

• Present and gain approval for care packages, arrange care home placements, and manage safeguarding concerns.

• Build and maintain strong partnerships with GPs, housing services, occupational therapists, and other stakeholders.

• Use case management systems such as Mosaic or Liquid Logic to document and track case progress effectively.
