What you will do:

- Conduct thorough assessments of individual needs to identify the best outcomes for service users, promoting independence and well-being.
- Coordinate with a range of services and initiate support plans to meet the identified needs of individuals.
- Champion a Strength-Based Approach to delay the necessity for long-term care, ensuring people can manage their own needs, risks, and uncertainties.
- Engage actively with community resources to maximise individuals' potential, utilising preventative and universal services in line with the Adult Social Care Strategy.
- Monitor ongoing service provision, ensuring effective coordination and focusing on optimal outcomes for individuals.
- Offer professional advice, building positive relationships with individuals, carers, colleagues, and community partners.
- Utilise knowledge of the Mental Capacity Act while undertaking safeguarding tasks to ensure effective protection of vulnerable adults.
- Maintain current caseload, continuously developing skills under supervision while providing mentorship to new or less experienced staff.
