• Complete Care Act assessments and reviews using a strengths-based approach.

• Conduct Mental Capacity Assessments and participate in Best Interests decision-making.

• Contribute to the team’s duty rota, responding to urgent queries and referrals.

• Support individuals with tailored care planning and ensure their needs are met in a person-centred manner.

• Collaborate with families, carers, and health professionals to achieve the best outcomes.

• Maintain accurate and confidential records in line with statutory requirements.

• Identify risks and implement appropriate safeguarding measures when necessary.
