Your Responsibilities

• Conduct Care Act-compliant assessments and reviews for adults with physical disabilities, frailty, or long-term conditions.

• Develop outcome-driven support plans that promote independence, autonomy, and wellbeing.

• Recommend appropriate equipment, assistive technology, or home adaptations to enable safe living environments.

• Promote reablement and strengths-based approaches to reduce reliance on long-term care services.

• Identify and respond to safeguarding concerns, performing risk assessments and applying preventative strategies.

• Collaborate with GPs, nurses, housing professionals, and social care colleagues to deliver coordinated care.

• Maintain accurate and up-to-date electronic case records, ensuring full compliance with HCPC and GDPR guidelines.

• Remain actively registered with the Health and Care Professions Council (HCPC) and engage in reflective practice, training, and team development.

About You

• Registered Occupational Therapist with the HCPC.

• Experience working with older adults, community rehabilitation, or social care settings preferred.

• Sound understanding of the Care Act 2014, strengths-based practice, and safeguarding principles.

• Strong interpersonal, analytical, and documentation skills.
