Your Responsibilities

Conduct Care Act-compliant assessments and reviews for adults with physical disabilities, frailty, or long-term conditions.

Develop outcome-driven support plans that promote independence, autonomy, and wellbeing.

Recommend appropriate equipment, assistive technology, or home adaptations to enable safe living environments.

Promote reablement and strengths-based approaches to reduce reliance on long-term care services.

Identify and respond to safeguarding concerns, performing risk assessments and applying preventative strategies.

Collaborate with GPs, nurses, housing professionals, and social care colleagues to deliver coordinated care.

Maintain accurate and up-to-date electronic case records, ensuring full compliance with HCPC and GDPR guidelines.

Remain actively registered with the Health and Care Professions Council (HCPC) and engage in reflective practice, training, and team development.
About You
-
Registered Occupational Therapist with the HCPC.

Experience working with older adults, community rehabilitation, or social care settings preferred.

Sound understanding of the Care Act 2014, strengths-based practice, and safeguarding principles.

Strong interpersonal, analytical, and documentation skills.
