What you will do:

- Lead and manage the Adults Hospital Discharge team to ensure exceptional service delivery.
- Develop and implement strategic plans to enhance service delivery, maintaining compliance with statutory guidelines and local policies.
- Collaborate with health partners, external agencies, and internal teams to ensure seamless transitions in care.
- Oversee assessments, care planning, and risk management for complex cases, ensuring the delivery of person-centred, strength-based approaches.
- Monitor and evaluate service performance, implementing improvements where necessary.
- Effectively manage budgets to align resources with service priorities.

Requirements:

- Qualified Social Worker with a current registration with Social Work England.
- Substantial experience in adult social care, particularly within hospital discharge and urgent care settings.
- Proven leadership experience with a successful track record in managing social care teams.
- In-depth knowledge of relevant legislation, policies, and best practices in adult social care.
- Strong communication, negotiation, and decision-making skills.
