An exciting opportunity awaits a passionate and experienced Social Worker to join the Children with Disabilities Team in the heart of Grimsby, Lincolnshire. You’ll be working within a supportive environment led by a highly experienced Practice Supervisor and Service Lead, alongside committed permanent staff members.

Key Responsibilities:

• Undertake Children and Families Assessments (CA 89), evaluating risk and support needs.

• Develop and implement Child in Need and Child Protection plans, chairing key meetings.

• Conduct assessments for and coordinate short break services for children with disabilities.

• Engage with children who may have communication difficulties, ensuring their voices are heard.

• Manage Children in Care cases, maintaining and reviewing care plans.

• Undertake court work and provide robust Transition Plans for young people moving into adult services.

• Ensure statutory Children’s Social Care requirements are met, including a strong understanding of EHCPs and SEND.

• Maintain a regular office presence (2–3 days/week) to fulfil statutory and collaborative functions.
