What you will do: -

• Deliver high-quality occupational therapy services, evaluating and addressing the needs of community patients.
• Collaborate effectively with multidisciplinary teams to create and implement personalised patient care plans. 
• Provide assessment, intervention, and evaluation of client progress towards identified goals.
• Maintain comprehensive and accurate patient records in accordance with NHS guidelines.
• Deliver tailored therapeutic interventions enabling clients to maximise their independence and quality of life. 
• Provide support and guidance for clients and their families, ensuring a comprehensive approach to care. 
• Stay informed about the latest developments in occupational therapy to continually enhance your practice.

Requirements:

• Candidates must be HCPC registered and experienced in working as a Band 6 within the NHS community settings.
• A valid driving licence is essential as this role involves travelling between various community sites.
