Qualified Social Worker - Advanced Practitioner - Adults Working Age Community Mental Health Team (CMHT) in Oldham, UK.

What you will do:

- Undertake Care Act assessments, arranging and monitoring care packages as necessary.
- Support individuals to remain well by continuously monitoring mental health status and needs.
- Collaborate as part of a multidisciplinary team (MDT) with health colleagues to manage risk effectively.
- Accurately record notes and case details on the Paris system to maintain up-to-date client records.
- Purchase care packages and manage safeguarding processes using the Mosaic system.
- Ensure all records are meticulously maintained in compliance with regulations and standards.

Requirements and qualifications:

- Qualified Social Worker status with relevant registration.
- Experience working within a community mental health team or similar setting.
- Strong understanding of the Care Act and safeguarding protocols.
- Ability to work collaboratively within a multidisciplinary team.
- Sound knowledge of using systems like Paris and Mosaic for case management.
- Three years of professional references and checks on employment history gaps are mandatory.
