What you will do:

• Manage a complex caseload, ensuring the comprehensive assessment, support planning, and review of unique individual needs, promoting autonomy and independence.
• Conduct thorough assessments using an array of health and social care tools, contributing notably to ongoing healthcare evaluations.
• Recognise and facilitate Reablement opportunities, guiding service access as required to maximise outcomes.
• Advocate a Strengths-Based Approach, encouraging individuals to assess their needs, manage risks, and reach both short and long-term goals while delaying prolonged care dependencies.
• Identify and promote the use of community and natural support systems, effectively utilising preventative services and funding resources in alignment with the Adult Care Strategy.
• Oversee and monitor the continuing provision of services, ensuring all stakeholder coordination with a focus on positive outcomes and alternative solution consideration.
• Liaise adeptly with local providers and support networks to achieve enhanced outcomes for service users.
• Offer detailed professional information, advice, and support to individuals, their carers, and colleagues, fostering positive relationships and partnerships.
• Undertake safeguarding work effectively in the role of Safeguarding Officer, ensuring protection and safety for vulnerable adults.
