What you will do:

- Conduct comprehensive assessments and develop tailored treatment plans for patients with respiratory and cardiovascular conditions, including elderly and critically care patients.
- Deliver high-quality, evidence-based physiotherapy interventions such as airway clearance techniques, non-invasive ventilation support, exercise prescription, and rehabilitation programmes.
- Independently manage a complex caseload, effectively prioritising patient needs while ensuring compliance with national and local guidelines.
- Contribute to the patient discharge process by liaising with community services to ensure seamless continuity of care.

Requirements:

- HCPC Registration along with minimum of 2 years' post-qualifying experience in a statutory social work setting.

- Full UK driving licence and access to a vehicle for work purposes
