What you will do:

• Ensure the timely identification and support of children in need, those at risk, children with disabilities, looked-after children, care leavers, and homeless young people according to Cardiff Children’s Service eligibility criteria and government guidelines.
• Assist individuals in overcoming difficulties and enhancing their independence through telephone discussions and appropriate referrals to both statutory and voluntary agencies.
• Contribute to developing efficient systems that ensure quality service provision across Cardiff and promptly identify individuals at particular risk.
• Collaborate positively with colleagues from both the statutory and independent sectors to ensure a cohesive multi-agency approach, empowering clients to advocate for themselves.
