Key Responsibilities

  • Implement a Reablement approach to support clients in regaining independence and confidence in their daily lives.

  • Conduct thorough assessments and provide tailored recommendations to support clients in remaining independent in their homes.

  • Manage a clinical caseload, offering guidance and supervision to junior team members where appropriate.

  • Contribute to the planning, delivery, and evaluation of service improvements and wider team development.

  • Accommodate flexible working hours, including possible extended shifts or weekend work based on operational needs.


Essential Requirements

  • HCPC registration (Health and Care Professions Council) is mandatory for this role.

  • A valid driving licence is required to facilitate travel across various community settings.

  • Previous community-based experience is desirable but not essential—enthusiasm for client-centred rehabilitation is key.
