Key Responsibilities

Implement a Reablement approach to support clients in regaining independence and confidence in their daily lives.

Conduct thorough assessments and provide tailored recommendations to support clients in remaining independent in their homes.

Manage a clinical caseload, offering guidance and supervision to junior team members where appropriate.

Contribute to the planning, delivery, and evaluation of service improvements and wider team development.

Accommodate flexible working hours, including possible extended shifts or weekend work based on operational needs.
Essential Requirements

HCPC registration (Health and Care Professions Council) is mandatory for this role.

A valid driving licence is required to facilitate travel across various community settings.

Previous community-based experience is desirable but not essential—enthusiasm for client-centred rehabilitation is key.
