What you will do:

- Prepare for social work contact by reviewing case notes, liaising with stakeholders, and evaluating information to ensure readiness and clarity in assessments.
- Conduct comprehensive assessments of support needs for Service Users, Carers, and Families, particularly in volatile or complex situations, assisting in the screening of referrals.
- Develop and review detailed Care & Support Plans for Service Users to address practical and emotional social care issues.
- Collaborate with Service Users, Carers, and Families to assist in making informed decisions regarding Self Directed Care and Individual Budgets, utilising the available resources efficiently.
- Actively engage with Safeguarding policies if abuse is suspected, and assess and manage risks to Service Users, fostering positive behaviour changes to reduce risk.
- Manage crisis situations with informed urgency, using appropriate legal interventions and maintaining high standards of data quality on social care systems.
- Ensure confidentiality of all personal information while participating in meetings or decision-making forums like Court appearances and Case Conferences.
- Advocate on behalf of Service Users, maintaining positive relationships with partnership agencies, adopting advances in working practices, especially ICT.
