What you will do:

Act as a lead practitioner within the integrated team, leveraging advanced knowledge and skills while working under the Mental Health Act and Mental Capacity Act frameworks.

Provide leadership and professional supervision to social workers, support workers, and support planners, ensuring excellence in service delivery.

Engage directly with adults with mental health needs, their families, and carers, offering support and co-ordinating complex care that requires advanced practice skills.

Conduct specialist assessments and act as a Safeguarding Adults Manager during investigations.

Champion personalisation across all community mental health teams, implementing a personalised approach to commissioning social care services.

Mentor staff, participate in audits, and support professional development programmes.

Collaborate effectively with internal and external colleagues, promoting equality and diversity within all aspects of your role.
