As an Occupational Therapist in our Reablement Team, you will be supported by a dedicated group of occupational therapy assistants and support workers who share your commitment to making a difference. You will lead the way in conducting strengths-based assessments and designing innovative plans tailored to meet each individual's goals.
What you will do:
- Conduct home visits to evaluate and assess the client's abilities and environment.
- Develop and implement bespoke reablement plans to enhance clients' independence.
- Collaborate with clients, families, and other health professionals to achieve desired outcomes.
- Monitor and review progress, adjusting plans as necessary.
- Provide guidance and support to occupational therapy assistants and support workers.
- Maintain accurate records and documentation.
Requirements:
- Hold a qualification in Occupational Therapy.
- Minimum two years post-qualification experience.
- HCPC registration is essential.
- Full UK driver's license to facilitate travel between client locations.
