What you will do:
- Lead a team of social workers and unqualified staff to support patient discharges from hospital settings.
- Ensure compliance with legislation including the Care Act, safeguarding, MCA, and BIM.
- Manage the flow of hospital discharges in a busy environment to ensure safe and timely transitions.
- Work collaboratively with partners, including the ICB, health, and third-party sectors.
- Attend and chair safeguarding meetings, court of protection sessions, multi-disciplinary teams, and other professional gatherings.
- Provide on-site support at the hospital and other bed base sites to ensure seamless operations.
Requirements:
- A social work qualification and strong understanding of relevant legislation are essential.
- Proven experience in supervising social workers and managing hospital discharge processes.
- The ability to work reliably and flexibly under pressure is crucial.
- A driver's licence and the capability to work across different sites are important.
