Key Responsibilities

• Conduct assessments of individuals and families to identify care needs and develop effective care plans.

• Collaborate with external agencies and service providers to deliver holistic support.

• Maintain accurate and timely case records in line with policies and procedures.

• Provide expert advice to service users and their families regarding available local services and resources.

• Work in partnership with fellow social workers and multidisciplinary professionals to implement sustainable community care strategies.

• Participate in regular training to maintain up-to-date knowledge of current legislation and best practice.

Required Skills & Experience

• 1–3 years' experience within a fast-paced neighbourhood team.

• Strong understanding and experience of working with LAS (Liquidlogic Adults System) is desirable.

• A full driving licence and access to a vehicle is advantageous.

• All employment gaps must be accounted for and verifiable.

• Degree in Social Work and current registration with Social Work England.
