Conduct assessments of individuals and families to identify care needs and develop effective care plans.
Collaborate with external agencies and service providers to deliver holistic support.
Maintain accurate and timely case records in line with policies and procedures.
Provide expert advice to service users and their families regarding available local services and resources.
Work in partnership with fellow social workers and multidisciplinary professionals to implement sustainable community care strategies.
Participate in regular training to maintain up-to-date knowledge of current legislation and best practice.

1–3 years' experience within a fast-paced neighbourhood team.
Strong understanding and experience of working with LAS (Liquidlogic Adults System) is desirable.
A full driving licence and access to a vehicle is advantageous.
All employment gaps must be accounted for and verifiable.
Degree in Social Work and current registration with Social Work England.
