What you will do:

- Support and maintain positive family and friends relationships by assessing and planning for children’s needs
- Develop clear and effective intervention plans in partnership with other teams and services
- Conduct home visits to ensure the safety and wellbeing of children and young people
- Collaborate with children, families, and outside agencies to achieve the best outcomes for children
- Maintain accurate and comprehensive case records and write high-quality reports
- Facilitate and attend team meetings as well as multi-agency meetings to discuss the needs of children and families
