What you will do:

• Serve as a Duty Social Worker for an Integrated Learning Disability Team, providing seamless health and social care services by collaborating with a multi-agency team.
• Record actions and decisions clearly and robustly using the Mosaic system.
• Provide direct advice and assistance when required, offering preventative support for adults with learning disabilities.
• Respond to urgent matters raised on duty, using a range of resources to ensure the safety and wellbeing of service users.
• Conduct assessments, reviews, and visits in response to emergencies, maintaining a first point of contact for safeguarding concerns.
• Ensure risk management and principles of the Mental Capacity Act are applied appropriately in social work practice.
• Deliver high professional standards of social care practice, including quality interventions, decision making, and adherence to the standards of Social Work England.
