What you will do:

• Conduct comprehensive assessments of patients within 72 hours of discharge from hospital.
• Collaborate with hospitals and community agencies to ensure seamless care transitions.
• Equip homes with necessary adaptive equipment to enhance patient independence.
• Maintain accurate and timely patient records.
• Travel between patients' homes, providing a high standard of personalised care.
• Start your day at our base by 8am, concluding at either a patient’s property or back at the base by 4pm.
