Social Worker - Children With Disabilities (CWD) Team based in Liverpool, earning a competitive £44.21 hourly rate. What you will do:

  • Assessment and Care Planning: Conduct detailed assessments of children and their families to pinpoint needs and develop bespoke care and support plans.
  • Advocacy and Support: Be the voice for children with disabilities, ensuring their needs and rights are central to all decisions.
  • Multi-Agency Collaboration: Partner with health professionals, educators, and other organisations to create comprehensive support packages.
  • Safeguarding: Proactively identify and address safeguarding issues, adhering to local procedures to protect the children involved.
  • Review and Monitoring: Continuously monitor care plans, making necessary adjustments to maintain or improve effectiveness.
  • Record Keeping: Keep precise and up-to-date case records and reports in compliance with both organisational and legal guidelines.
  • Family Engagement: Offer advice, information, and access to local resources to empower families.
  • Professional Development: Stay informed on best practices, legislation, and policies pertinent to children with disabilities and social work.


Person Specification:

  • Qualified Social Worker with a recognised social work qualification such as DipSW, CQSW, or equivalent.
  • Mandatory registration with Social Work England.


Experience:

  • Significant experience working with children with disabilities and their families.
  • Strong command of child development, disability issues, and safeguarding.
  • Proven experience in multi-agency collaboration and care planning.

Working Conditions:
