What you will do:

• Manage a caseload of children and young people with severe and profound disabilities, ensuring their needs are met effectively.
• Complete Child and Family Assessments, supporting families through Child in Need Plans, Child Protection Plans, and Children in Care plans.
• Conduct regular visits to children, young people, and their families, offering guidance and support as needed.
• Chair and participate in multi-agency meetings, fostering collaboration and ensuring all information is shared efficiently.
• Develop SMART Child Plans that aim to achieve the best outcomes for children and their families.
