What You Will Do:
- Conduct comprehensive assessments and create tailored treatment plans for adults within the community setting.
- Implement and review rehabilitation programmes to promote independence and well-being.
- Work collaboratively with a multidisciplinary team to ensure holistic patient care.
- Maintain up-to-date patient records and provide detailed reports for ongoing care strategies.
- Engage with patients and their families to educate and empower them in the management of their conditions.
To be successful in this role, previous experience working within a community occupational therapy service is essential. You will need to be HCPC registered and have a deep understanding of assessment and treatment interventions within a community setting. A car driver is also required to cover the wide-ranging community areas.
