What you will do:

- Undertake strengths-based assessments of needs for individuals with learning disabilities, focusing on their capabilities and aspirations.
- Develop, implement, and review personalised care and support plans in collaboration with service users and their support networks.
- Coordinate with health professionals, including community nurses, occupational therapists, and psychologists, ensuring a holistic approach to care.
- Ensure compliance with safeguarding procedures and actively participate in safeguarding enquiries when necessary.
- Conduct Mental Capacity Assessments and support Best Interest decision-making processes.
- Manage a complex caseload utilising professional supervision and reflective practice for continuous improvement.
- Advocate for service users' rights, choices, and access to opportunities such as employment, housing, and social inclusion.
- Maintain accurate and timely case records on electronic systems, ensuring seamless information flow and documentation.
- Attend multi-agency meetings and contribute to Education, Health and Care Plans (EHCPs) or Continuing Healthcare (CHC) assessments when required.
