What you will do:

• Assess patients’ needs to ensure safe and timely hospital discharge planning.
• Collaborate with healthcare professionals, patients, and their families to establish comprehensive discharge plans.
• Identify and coordinate community resources and services to support patients post-discharge.
• Provide both emotional and psychological support to patients and their families during transitional care processes.
• Maintain accurate and up-to-date documentation and reports as required by the hospital discharge team.
