This position offers a competitive rate of £26 per hour and welcomes both full-time and part-time candidates for an ongoing assignment of 3-6 months. What you will do:
- Assessment & Care Planning: Conduct comprehensive mental health and risk assessments, and develop, review, and implement personalised care plans tailored to each individual's needs.
- Case Management: Take charge of a caseload of service users with complex needs, adhering to the Care Programme Approach to ensure everyone receives the attention and support required.
- Clinical Interventions: Deliver top-notch evidence-based nursing care, including medication management, crisis intervention, and therapeutic support to promote mental wellbeing.
- Multi-disciplinary Working: Collaborate seamlessly with professionals across various sectors to provide coordinated, holistic care that benefits both the individual and their family.
- Risk & Safeguarding: Astutely identify and manage clinical risks, ensuring all safeguarding protocols for adults and children are diligently followed.
- Documentation & Compliance: Maintain accurate clinical records and ensure compliance with all relevant legal, ethical, and professional standards.
