What you will do:
- Conduct assessments and develop support plans in line with statutory guidelines.
- Provide ongoing support and advice to individuals and their families.
- Collaborate with a range of partners, including health services and community organisations, to ensure a holistic approach to care.
- Record all case observations accurately in the Local Authority's social care database.
- Participate in the duty rota, responding to emergencies and prioritising tasks efficiently.
- Apply relevant social care legislation in your practice, ensuring compliance with statutory responsibilities.
- Foster an environment of anti-oppressive practice and equal opportunities.
The knowledge:
- Expertise in relevant adults social care legislation including The Care Act 2014, Mental Capacity Act 2005, Deprivation of Liberty Safeguards 2015, Mental Health Act 2007 (Amended), Continuing Health Care, and Section 117.
- Ability to interpret and apply social care legislation to practice while managing workloads and prioritising tasks effectively.
- Strong IT and communication skills to effectively manage and record data.
- Proficient in working collaboratively within a team and independently.
- Committed to person-centred, strengths-based social work practices.
The experience:
- Experience working with adults with learning disabilities, mental health needs, and physical disabilities.
- Skilled in undertaking assessments and identifying necessary services.
- Understanding of the reviewing and monitoring process, alongside managing risks and participating in safeguarding procedures.
The qualifications:
- Qualified Social Work degree.
- Current registration with Social Work England.
- Current full driving licence and access to mobility support.
- Willingness to participate in further training as required by the Local Authority.
