What you will do:

  • Act as the first point of contact for service users, providing them with information, advice, and support related to drug and alcohol problems.
  • Participate in the multi-disciplinary team to deliver community-based care in line with individualised care plans.
  • Screen new referrals and assess eligibility for services, prioritising based on need and risk.
  • Manage your own caseload of patients with low complexity issues, offering interventions, support, and education.
  • Assist with community detoxification and generate relapse prevention strategies.
  • Liaise effectively with other professionals to ensure comprehensive care and the sharing of information regarding patient progress and risk.
  • Facilitate and deliver therapeutic activities tailored to the diverse needs of patients.
  • Prepare and maintain accurate records of all patient interactions and interventions using the Health and Social Care electronic database system 'RiO'.
  • Communicate effectively with patients, families, carers, and partner agencies to enhance service delivery.
  • Provide education and awareness programmes on drug and alcohol misuse for the community, enhancing public knowledge and preventive measures.
