An exciting opportunity has arisen for an experienced Qualified Social Worker to join the Adult Community Health Centre in Leeds. This role offers flexibility with both full-time and part-time options available—including the opportunity to work 18.5 hours across three days. Key Responsibilities

Complete Community Care Assessments, including CHC Checklists and DSTs, to support the CHC team during maternity leave cover.

Manage a caseload using a person-centred approach to assess the needs of service users and carers in line with statutory frameworks.

Analyse complex information and develop service plans that meet assessed needs and comply with relevant legal and organisational standards.

Conduct safeguarding enquiries and manage risk using the appropriate frameworks, ensuring the safety and wellbeing of vulnerable adults.

Collaborate effectively with service users, carers, families, and professionals to ensure coordinated, high-quality care.

Assist in screening referrals, planning care packages, and contributing to commissioning processes.

Maintain accurate and up-to-date case records and produce professional reports, including evidence for legal proceedings when necessary.

Participate in team meetings, training sessions, and ongoing practice development to ensure alignment with current policies and procedures.
