What you will do:

• Develop person-centred, multi-agency support plans tailored to individual needs.
• Provide assertive and persistent support to ensure effective engagement and help individuals achieve personal goals.
• Act as the single point of contact, flexibly managing and planning interactions to address unique client needs.
• Offer initial advocacy support to secure access to relevant services and agencies.
• Gradually reduce intensity of support as clients gain independence and control over their lives.
• Collaborate with multi-agency teams to regularly review and address barriers to progress.
• Encourage participation in meaningful activities to foster inclusion and social connection.
• Identify and work through potential crisis points that may hinder engagement and success.
• Plan exit strategies to secure continuous support and integrate aftercare provisions.
• Work with service users to record and maintain their progression and support plans.
• Identify and therapeutically address risks associated with clients' complex needs.
• Contribute to project evaluations by documenting activities and outcomes accurately.
• Engage with individuals who struggle to access appropriate services due to complex needs.
• Build trust and rapport with service users and support agencies, serving as a crucial support link.
• Co-ordinate and review person-centred support plans in collaboration with multiple agencies.
• Facilitate effective engagement with services to meet clients' needs and ambitions.
• Establish and sustain strong partnerships with various organisations and services.
