What you will do:

• Take charge of an allocated caseload, ensuring all patients receive the highest standard of care and attention.
• Conduct thorough mental health assessments, care act assessments, and develop detailed care plans tailored to individual needs.
• Administer medication responsibly and accurately while maintaining up-to-date documentation including tribunal reports and progress notes.
• Engage in CPA and CTO reviews, offering social care support and conducting risk assessments to safeguard patient well-being.
• Exhibit excellent interpersonal and communication skills as you advocate for patients and collaborate with a multi-disciplinary team.
• Organise your own caseload efficiently, demonstrating resilience and the ability to work under pressure in a fast-paced environment.
