What you will do:

• Independently assess, diagnose, and manage patients, prescribing appropriate medications within your competence.
• Collaborate with multidisciplinary teams to ensure safe and effective care delivery throughout the Trust.
• Provide specialist advice and education on medicines management to patients, carers, and healthcare colleagues.
• Maintain accurate clinical documentation and comply with Trust prescribing policies and procedures.
• Engage in audits, clinical supervision, and professional development to strengthen your clinical skills and knowledge.
• Drive service improvement initiatives and promote evidence-based practice within your clinical area.
